You can leave their Vestiges on the ground and choose to walk past the successor instead of helping them in their respective memory. To obtain the 'Heirs' ending, you will need to avoid restoring the memory of each of the four successors. None of them not tied to successors are missable and only four are tied to a different ending. You can use this playthrough to obtain as many blood codes as possible. This may sound a bit confusing so please see the guide for more information. This will give you the true version of this ending, which is very similar to the good ending. Once you have done so, simply defeat the final boss once more and repeat the above steps to leave Home Base. Now, travel back to Home Base and restore Io's memories. Once the credits have rolled once, you can continue your game and you will be back at the last Mistle before the final boss. Now you can leave through the back door and trigger the final cutscene. If done correctly, you will be back in Home Base with everyone you saved. This will allow you to get the good version of this ending first, after defeating the final boss. While you do this, make sure not to restore all of Io's memories (called Eos Vestiges). This will allow you to restore the successor's memories after defeating the boss. You will find an Attendant of the Relics outside of each boss arena and will need to restore the specific area's Vestiges before defeating the boss. What you will need to do is collect all successor Vestiges in four areas: Cathedral of the Sacred Blood, Ridge of Frozen Souls, City of Fallen Flame, and Crown of Sand. As mentioned above, you can save yourself a playthrough by earning both variants of the 'Dweller in the Dark' ending on the same playthrough. There are a lot of miscellaneous achievements you can tackle at any time so feel free to do them on this playthrough. These can also drop, albeit very rare, from a few enemies outlined in the guide. One medal is obtained per boss defeated in co-op and two are available as world drops per playthrough. There is also a very grindy achievement that requires 400 Mark of Honor medals. Technically, the five achievements for all endings and all blood codes are missable on any given playthrough but you can cleanup achievements before choosing to go to NG+ after the game has been beaten. Nearly all achievements can be earned on a single playthrough.
